4a. Being a hybrid program, SBH partners with parents in the education of their children. This means that parents will need to commit to between 5-10 hours a week giving structure and direction for their children at home. Is this something you have any reservations about? Please explain.

4b. Participating in hybrid education requires the parents to have a comfort and familiarity with regularly going online to see assignments and communicating with teachers. Is this something you have any reservations about? Please explain.

10. Having parents join our teaching team is needed for the sustainability and life of our program. Are you interested in joining us as a tutor in the program? Grant For consideration which I acknowledge, I irrevocably grant to St. Benedict Hall the right to use my student’s image in all forms and media including composite or modified representations for the purposes of advertising/marketing and our newsletters. I waive the right to inspect or approve versions of my image used for publication or the written copy that may be used in connection with the images. Release I release St. Benedict Hall from any claims that may arise regarding the use of my image, including any claims of defamation, invasion of privacy, or infringement of moral rights, rights of publicity, or copyright. St. Benedict Hall is permitted, although not obligated, to include my name as a credit in connection with the image. *TO MAKE THIS APPLICATION COMPLETE, PLEASE INCLUDE:

• A sample writing from your children. For Form 1B sample should include: Write their names, and the alphabet. For Form 2, 3, and 4: Write a paragraph about themselves (interests, family, etc.). Please include this writing sample when turning in this application. This will help us appropriately place your child in the Form where they will learn best.

Before starting classes at SBH we ask that you share a copy of your letter of intent. According to Hawaii State Law: “The parent must provide the local public-school principal with a notice of intent to home educate the child before initiating homeschooling. The notice of intent may be submitted on a HIDOE-developed form (Form OIS-4140) or in a letter containing the following items: name, address, and telephone number of the child; birthdate and grade level of the child; and signature of the parent (Hawaii Administrative Code (HAC) §8-12-13). The notice of intent must be acknowledged by the principal and the district superintendent. The notice of intent is used for record-keeping purposes and to protect families from unfounded accusations of educational neglect or truancy. HAC §8-12-13(b).”
