We are thrilled to offer your child many experiential activities they would not get in any other educational setting.  Through time in Nature, gymnasium, movement, and free play we allow your child to stretch their comfort zones and grow while in a loving community.  However, there are inherent risks involved in this type of education and setting, and Saint Benedict Hall cannot be held liable for such accidents and injury.

Informed Consent/General Release

By Signing Below I/We:

- I/We the parent(s) hereby give my/our approval to for my/our child to participate in any/all activities during the school year.  I/We will notify the director/staff of Saint Benedict Hall should I/We chose for my/our child to avoid specific activities for any reason.

- I/We assume all risks and hazards incidental to such participation with Saint Benedict Hall, including, but not limited to, transportation to and from school, field trips and activities, accidents, injury or death while at Saint Benedict Hall or while my/our child is in the care of tutors/staff of Saint Benedict Hall.  I/We do herby waive, absolve, indemnify and agree to hold harmless Saint Benedict Hall and their staff and guest participants and persons supervising/teaching my/our child.

- I/We understand medical insurance will NOT be provided by Saint Benedict Hall and all medical costs incurred while in the care of Saint Benedict Hall is my/our responsibility.

- I/We agree in the event of an emergency that requires immediate action before I/We can be reached for a staff member of Saint Benedict Hall to use their best judgement in the treatment/care of my/our child, including but not limited to, CPR, bandaging, taking your child to the emergency room and administering medication to treat severe acute conditions such as allergic reaction, seizures, etc. 

SBH Waiver of Liability

Since participation in nature and activity during gymnasium can be dangerous, Saint Benedict Hall requires that all participants (and their adult parent(s) or guardians) to assume all risks associated with the school year by signing this general release.

For and in consideration of my child being permitted to participate in Saint Benedict Hall activities, I hereby voluntarily release, discharge, waive and relinquish any and all claims or actions for damages for personal injury, permanent disability, death, or property damage which I or my child may have, or which may hereafter accrue to me or my child, as a result of my participation in Saint Benedict Hall activities during the school day and while I or my child are at the facility, on field trips, or off campus in a school related activity or for any other reason. This release is intended to discharge, in advance, Saint Benedict Hall its staff, director, and board, and the substitute tutors/volunteers from any and all liability arising out of or connected in any way with my child’s participation in the school years activities.

I further understand that serious accidents occasionally occur and that participants occasionally sustain serious personal injuries, death or property damage as a consequence thereof. Knowing the risks, I have voluntarily applied for my child to attend Saint Benedict Hall and participate in the activities of the curriculum and thereby agree to assume those risks to release and hold harmless Saint Benedict Hall, staff, director, and board, or substitute tutors/volunteers, who (through negligence or carelessness) might otherwise be liable to me or to my child (or my heirs or assigns) for damages.

I further understand and agree that this release, discharge, waiver, and assumption of risk is to be binding on my and my child’s heirs, executors, administrators, and assigns.

I further agree to indemnify and to hold harmless Saint Benedict Hall, staff, director, and board, or substitute tutors/volunteers for activities, for any loss, liability damage, cost or expense which may incur as a result of any injury or property damage I or my child may sustain while participating in a SBH activity.

I agree to comply with the program’s stated and customary terms and conditions for participation according to Saint Benedict Hall.

I have read the SBH Informed Consent/General Release & Waiver of Liability, fully understanding its terms, that I give up substantial rights by signing it, and sign it voluntarily.
